Register mobile device after QR scan
POST/verification-actions-srv/setup/:method/scan
Called by the mobile app after scanning the enrollment QR code. Links the device and push token to the in-progress exchange_id from initiate.
Public: no bearer token. Missing or invalid fields return 417, not 401.
