Replace CIMD policy

Replaces the CIMD app-management policy (policyType=cimd).

CIMD is replace-only: the default policy must already exist (created by list/GET when needed). A missing policy returns 404 APP10125. CIMD disabled returns 404 APP10153.

Validation failures return 400 with CIMD-specific codes (APP10154–APP10160, APP10165).

Requires scope cidaas:apps_write and role ADMIN or SECONDARY_ADMIN.
